- Tier 3

Fifteen (15) groups sold with a minimum of 75 enrolled lives combined
- Tier 4

A $500 Amazon gift card will be awarded to the Agent with the most premium sold during the sales incentive program.
The incentive program will be for all group plans sold with effective dates of October 1, 2019 through January 1, 2020
Awards will be distributed no later than February 15, 2020.
